If you'd prefer to send a DVD, VHS, Beta or MiniDV, follow these steps:
- Cue up the tape.
- Describe the play on paper.
- Give us the names of the players, coach, team and/or opposing team, and school, if applicable, with any awards or honors they won.
- Date the play took place.
- Get your parents to download, print out and sign our Parental Consent Form.
Then, pack it all up and ship to:
